How to complete an Application to Import Disability Assistance Dogs
The Application to Import Disability Assistance Dogs consists of the following sections:

Importer details
Enter the name, street address, telephone number, fax number and email address (if applicable) of the person or organisation importing the goods into Australia. In the space provided, enter the contact person's name for this consignment. This name will be included on the Import Permit.
Exporter details
The exporter is the individual or organisation who is sending the commodity to Australia. Provide the exporter's street address, telephone number, fax number and email address (if applicable).
Where guidelines allow sourcing of the same commodity from more than one supplier in the country of origin, you may nominate 'various' as the exporter. In these cases, address details are not required for each supplier.
Animal details
Animal name
Please indicate the name of your dog.
Age or date of birth
Please indicate your dog's age or birthdate.
Breed
Please indicate your dog's breed, or for mixed breed animals, indicate the breed/s which your animal most closely resembles.
Sex
Please indicate the sex of your dog by ticking the appropriate box. If pregnant, please indicate this by ticking the appropriate box and indicating the expected date of birth.
Ongoing veterinary problems/medication requirements
Please indicate if your dog has any ongoing veterinary problems or requires medication. Please provide details if necessary.
Microchip details
Please provide your dog's microchip number and indicate the type of microchip by ticking the appropriate box.
Other details
Country of export
As each country has a different pest and disease risk status, information is to be provided as to the country of export of the live animals or reproductive materials.
Australian port of arrival
Please indicate the port in which your cat or dog will arrive in Australia.
Estimated date of arrival
Please indicate estimated date that the live animals or reproductive materials you intend to import will be arriving in Australia.
Supporting documentation
Please attach supporting documentation and confirm the required documents are included by ticking the appropriate boxes.
Importer declaration
The importer declaration outlines the legal obligation of the individual that is applying for the Import Permit and must be filled in by all applicants. Please read carefully before signing the declaration.
Payment details
Please note: In accordance with the Quarantine Service Fees Determination 2005, Import Permit fees do not apply for Application to Import Disability Assistance Dogs.
