Application Classifications

  1. To be ticked when applying for a new Quarantine Approved Premises that is not co-located with an existing premises.
  2. To be ticked when applying to change the Senior Managers name or an accredited/contact person’s.
  3. If you wish to apply for a co-located premises, you will need to fill in Schedule One rather than this form. Schedule One is significantly shorter and will save you time.

Note: In most situations you must have at least one Accredited Person, talk to your regional office about accreditation training.

Applicant Details

  1. Legal entity is generally taken to mean "an individual person or a group of persons, a registered partnership, or an Australian registered company, or government body" who can be held to be legally accountable for the operations of the premises, including debts incurred. Please include with your application, a certificate of incorporation or written evidence of partnership or reference to the act or royal charter that your organisation was created under. For further clarification contact your Approved Premises Officer (APO) in your region.
  2. Alternative trading names are those names your organisation has had registered with your local state body. Please include copies of any business name registration certificates for any alternative names listed.
  3. Provide the applicant’s postal address. This may be a PO box or street address. Ensure all contact details provided are general details and not specific to a person.

Proposed Place

  1. Provide the actual street address of the place to be approved, including room numbers or area names if applicable. This cannot be a PO box number. If possible, ensure all contact details provided are general details and not specific to a person. The physical postcode may require that additional information is submitted in support of the Application. Refer to application FAQs.
  2. This is the material you plan to import to the QAP. Please list full scientific names and/or details of goods to be imported. If you currently hold permits and plan to import the material listed on those permits to this proposed QAP please list the permit numbers.
  3. Deposited Plan/Folio Number is the number provided by the lands department in your state. It shows the exact size and location of your property in relation to other properties.
  4. If the premises is already AQIS approved for export please provide the details here. (More information is available from AQIS exports)
  5. The questions in this section only apply to certain classes of Quarantine Approved Premises, usually class 1 and 2. Please ensure dimensions are listed in metres.
  6. If the premises has ever been approved by AQIS in the past as a Quarantine Approved Premises, please locate the old Approval number and include it here. The number would be structured as state letter followed by 4 digits e.g. NSW=N0000, QLD=Q0000, NT=C0000.

Occupier Details

  1. Freehold: Property that is owned by the applicant. Lease: Contract granting use or occupation of property during a specified period in exchange for a specified rent.
  2. Please provide the details of who owns the leased property.
  3. & 4. If you share your premises, does the registered company/person you share with currently have a Quarantine Approved Premises? If so, please provide their name and quarantine approval number, so AQIS can ensure there is no risk of cross contamination.

Management Details

  1. The Senior Manager is the individual primarily responsible for compliance with AQIS requirements. The Senior Manager will be called to explain any non-compliance with AQIS requirements. The details provided should be as specific as possible eg. the actual extension number of the Senior Manager.

    Accreditation number: This is the number you receive once you have completed the AQIS approved training. There must be at least one Accredited person per QAP.

  2. & 3. Accredited/Contact persons are people AQIS can contact to discuss issues or day to day needs. There should be two people or more and they should be aware of AQIS requirements. Accredited/Contact Person One and Senior Manager may be the same Person, but it is a good idea to have these as different people so as to provide a further contact point for AQIS in case of emergency.

    The Senior Manager and Accredited/Contact persons names must be proper names. Nicknames, abbreviations and aliases will not be accepted.

Personnel History Questions

  1. When deciding whether to approve a place as a QAP, AQIS is required to take into account whether the applicant and any person nominated in Section D has been convicted of certain offences. Please answer each question for the applicant and each person nominated in Section D.

Suspended: Temporarily lost approval

Revoked: Where a registered Quarantine Approved Premises was cancelled

Other approvals: such as Co-regulation

Conditions of Approval

These conditions are the basic and preliminary conditions you must agree to. AQIS will require you to meet other conditions before you can become fully registered as a Quarantine Approved Premises.

Declaration

If the Senior Manager makes a false declaration the Senior Manager/applicant may be subject to the following:

  • Quarantine Approved Premises cancelled
  • Subject to criminal prosecution under Section 136.1 of the Criminal Code Act 1995.

It is important that the Senior Manager completes the application as accurately as possible, as well as ensuring the people who are listed in Section D have been asked the questions in Section E.
