Third Party assessment and Co-location of Class 5 Quarantine Approved Premises
Since its implementation the co-location policy has provided benefits to institutions operating multiple facilities. Although co-location can be undertaken for all classes of QAP, it is of particular relevance to class 5 operators as the implementation of the new containment criteria requires approval of the space (room/laboratory) to a specific level of containment. As a result there are potential financial advantages, especially for universities and non-profit research institutions that may currently have multiple rooms registered under a single approval. To assist premises in determining whether or not they need to deconsolidate existing approvals that cover multiple rooms/laboratories to comply with the new QC criteria, a Guide for Operators has been produced and can be accessed below.

Deconsolidation of Existing Class 5 QAP Approvals - A Guide For Operators
This document outlines the principles behind the deconsolidation of existing QAP approvals that cover multiple rooms/laboratories and assists operators to make decisions on whether or not they require additional approval of laboratories to be used for the handling of quarantine material in order to achieve compliance with the new class 5 QC criteria.
Deconsolidation Examples - Operator
To obtain compliance with the new Class 5 QAP Quarantine Containment (QC) criteria it is a requirement for all QAP's of QC level 2 to 4 to have a premises inspection undertaken by an AQIS approved Third Party Assessor. Documents are available on this page that are to be used when notifying AQIS of your premises Third Party assessment booking details along with checklists to be completed when forwarding assessment reports and interim approval submissions, where applicable.

This booking form is to be completed by the AQIS Third Party Assessor upon confirmation of a booking for assessment of a Class 5 QC 2 to 4 facility.

Premises Refurbishment or Rebuild Operator Checklist
This checklist is to be completed by operators of class 5 QC 2 to 4 premises who
- have not passed their third party assessment and intend to refurbish the existing premises ; OR
- have not passed their third party assessment and intend to build new compliant facilities; OR
- knowingly fall short of the criteria and consequently, a third party assessment has not been conducted and new compliant facilities are to be constructed.
This checklist must be provided, together with the documentation specified, to the QAP Section in the region in which the QAP operates. Addresses for the QAP section in each region are provided with this checklist.

Premises Low or Medium Level Priority Failure Checklist
This checklist is to be used by operators where the premises quarantine containment level is 2 to 4, and the QAP has passed the third party assessment overall but has not met an;
- ‘AQIS Medium Priority’ criterion; OR
- ‘AQIS Low Priority’ criterion;
This checklist will need to be provided, together with the documentation specified, to the QAP Section in the region in which the QAP operates. Addresses for the QAP section in each region are provided with this checklist.
