Request for permission to destroy form

An importer/agent may elect to have goods destroyed at any point in the importation process.  If an importer/agent is electing to have goods destroyed, a signed Request for Permission to Destroy form stating they are the owner or an agent of the owner with authority to destroy the goods; and that they are requesting that a Quarantine Officer give permission to destroy the goods must be provided.
