This manufacturer's questionnaire is for the following imported products:

Animal feed (stockfeed, stockfeed supplements, pet & fish food, aquaculture feed)
Plant-based fertilisers

• This questionnaire must be completed by the manufacturer of the product intended for export to Australia.
• This must be submitted with the application for a permit for import of animal feed or plant-based fertilisers into Australia.
• The information collected will be used to determine import conditions for these products.
• Please use additional paper if insufficient space.
