Please use this form to advise AQIS of any changes to previously reported crew/passenger movements, human health and waste disposal information  – only complete the relevant section of this form to report information that has changed.

 Please note: the form must be provided to AQIS 12-96 hours prior to the vessel’s arrival in the next port.

For visits to non-proclaimed ports, refer to your ship’s agent for the responsible AQIS regional office.
